How does a demand pacemaker know when to deliver a stimulus ? Does it recognize the ECG ? The pacemaker does not sense the surface ECG nor the ECG near the pacemaker can. It detects what is going on inside the heart itself by measuring the potential difference (voltage) between the two electrodes used for pacing. The voltage of intracardiac depolarization is larger than that of the surface ECG and is called an electrogram rather than electrocardiogram. Therefore, the time of electronic sensing the intracardiac electrogram does not correspond with the onset of the surface ECG because it takes a finite time for the activation to reach the electrodes (for example in the right ventricle) and generate the electrogram pacemaker time of sensing indifferent plate Surface ECG time Ventricular electrogram tip electrode time sensing level Voltage or potential difference between tip-electrode and indifferent plate A.
